To attend the conference, you will need to complete Application Form VAF1C - Business Visitor.  To download this form, please click here .

It is recommended that you apply for your visa well in advance (at least 6 weeks before the meeting).

The Society is happy to provide you with documentation to support your application.  However you must register to attend Physiology 2011 before we can provide this.

Please send the following information to Sarah Bundock

• Full name
• Date of birth
• Address
• Telephone number
• Passport number
• The purpose of the trip (i.e., to attend Physiology 2011).
• How you intend to stay in the UK. 
• Details of your accommodation
• Details of how your living expenses will be covered
• Date you intend to leave the UK
